CMS-1500 Reference Manual

 

Much of the patient record screen is laid out to resemble the CMS-1500 claim form. This layout leaves no question about where the information printed on the form comes from.

 

 

Patient's Name [Box 2]

The patient name is actually three fields, the patients first and last names and a middle initial. The middle initial holds only one character so you cannot enter a period following the initial. Aeris will take care of that for you when the patient's name is printed.

 

Patient's Birth Date [Box 3]

Enter the patient's date of birth. You can right-click or press F2 in this field to select a date from a popup calendar.

 

Sex [Box 3]

Select the check box for either Male or Female.

 

Patient's Address [Box 5]

The patient's address is actually four fields, the street address, the city, the state code, and the zip code.

 

Telephone [Box 5]

The telephone number is two fields, one for the area code and another for the telephone number itself. The area code will automatically complete when filled and the cursor will jump to the telephone number. You don't need to enter the hyphen in the telephone number; Aeris will do that for you when you tab away from the field.

 

Patient Status [Box 8]

There are six check boxes in this group but they correspond to three subgroups and they will be discussed separately.

 

The three check boxes along the top are for the patient's marital status. You can have only one of these three boxes checked at a time so if you change a patient's marital status from single to married, for example, checking the Married box will uncheck the Single box. If you click on a box that is already checked, the check goes away and none of the boxes will be checked when the form is printed.

 

The check box for Employed stands by itself. Check it here and it will be checked when the form is printed.

 

The final two boxes are a group and work like the marital status check boxes. Checking one box clears checks from the other. Clicking on a box that is already checked will uncheck that box without placing a check the other box.

 

Patient's Account Number [Box 26]

This is the patient ID in Aeris. It uniquely identifies the patient. You can use numbers, letters, or other characters. A practice preferences option lets you tell the program whether it should assign the next highest number when a you add a new patient. If the practice option is set to automatically assign Patient IDs and you are not a system administrator, then this field will not be available to edit. If the practice option is set to automatically assign Patient IDs and you are not a system administrator, then this field will not be available to edit.

 

Service Facility Location [Box 32]

The Facility in box 32 is where you treated this patient. You have two options for this group of fields: you can select a facility from the drop-list or enter them directly. If you want to use a facility from the list but the facility is not listed, you can press the ellipses button to add a new facility to the list. Entering a facility directly is recommended only if you are seeing a patient in an unusual location. Otherwise selecting a facility from the list means that you need to enter the facility only once.

 

Most of the time the facility will be your office and so a practice option lets you set the default facility for new patients. When you add a practice, the practice is also added to the facilities list to save you time.

 

Principal Provider

Use the drop-list to select this patient's principal provider. The principal provider will be the default provider whenever you add a service for this patient.

 

Social Security Number

This is often needed by insurance even though there is no standard place for it on the form.

 

Account Balances

The balances block shows you the current patient and insurance balances on this patient's account and the balances as of the last statement.
